It is a genuine novelty, not a scenic volcano in the usual sense; treat it as a half-day, not the trip’s centerpiece.

Is three days enough to add Volcan del Totumo?
Yes, comfortably. Totumo is a half-day activity even with the drive included, so it slots into a third day without crowding out rest time. What it does not leave room for is La Boquilla or San Basilio de Palenque; if either interests you, move to the 4 day or 6 day version.
Is Volcan del Totumo worth a full day, or just a stop?
Just a stop. The mud float itself takes 30 to 45 minutes, the lagoon rinse another 15, and the round-trip drive is the real time cost. Book a morning tour, be back in Cartagena by early afternoon, and use the rest of day three to rest before your onward journey rather than padding the schedule around a novelty.
