Day 1: Arrival and walled-city orientation
Fixed-rate kiosk taxi from CTG (20,000 to 30,000 COP) into Getsemani or the Walled City, then walk the Old City’s plazas during the cooler afternoon hours, cathedral, Plaza Bolivar, Las Bovedas’ artisan market, all inside the UNESCO-listed walled core. Save the ramparts near Baluarte Santo Domingo for sunset, Cafe del Mar or a street beer on the same wall, then an early dinner. Four days still gives the coast most of the itinerary; keep this one contained.
Day 2: Rosario Islands or Playa Blanca
Boats leave Muelle de la Bodeguita around 8am. The standard trip runs 159,000 COP for an adult and 139,000 COP for a child, with a separate 25,000 to 29,000 COP national park and dock tax paid in cash at the pier, often not included in the advertised price. Book a reviewed operator over a dock tout, the overselling reputation here is specific and well documented (the current UK FCDO advisory covers the same ground), and plan on being back in Cartagena by evening.
Day 3: Volcan del Totumo
An early departure (8 to 11am) beats the heat and the tour-bus crowds. Entry runs 20,000 to 25,000 COP at the site, an organized round-trip tour from Cartagena runs 35 to 65 USD, and the mud float plus lagoon rinse takes under an hour total. Treat it as a half-day novelty, tip 5,000 to 10,000 COP for the rinse assistance, and be back in the city by early afternoon.
Day 4: La Boquilla
This fishing village turned beach club and kitesurf spot sits about 20 minutes north of the center, a genuinely lower-key alternative to a second Bocagrande afternoon. Spend the morning on a mangrove canoe tour or a kitesurf lesson, roughly 50,000 to 150,000 COP depending on what you book, then a beach club lounge chair and lunch for the afternoon. Return to Getsemani for a final evening out.

Is La Boquilla worth it over a second Bocagrande afternoon?
Yes. Bocagrande’s own beach is mediocre and crowded with vendors; La Boquilla trades that for a working fishing village with mangroves, kitesurfing, and a fraction of the foot traffic. It is the better fourth day for anyone who has already done the islands and does not need a second, weaker version of the same thing.
Is four days enough to skip Bocagrande entirely?
Yes, and this itinerary does exactly that. Bocagrande earns its keep as a convenient base with pharmacies and big hotel brands, not as a beach destination, and four days has better uses for its one open slot. If you specifically want Bocagrande’s shopping strip, swap it in for La Boquilla on day four.
